If I signed an insurance waiver when I was initially hired 4 years ago, can I request insurance now that my spouse is retiring?

Unless the terms of the waiver said that you would have the right to get insurance later if your spouse retires (or if you lose insurance provided through some other source, such as your spouse's job), then no--you would still be contractually obligated to the waiver you signed. That does not mean you cannot discuss the matter with your employer, and they may choose to let you get insurance--possibly if you are willing to give up some salary or other benefit to balance the cost of company-provided insurance.
