As a general matter, you would need something in writing from an officer or representative of the organizations under/through/with/etc. you completed the community service, indicating the dates, hours, and nature of the service you completed and that said service was satisfactory. However, it would be a good idea to contact the court and/or any officer put in charge of monitoring your compliance, to see if there is preferred or specific form you need to have completed.
