What is required by law to be included in my personnel file?

I requested my file after I parted ways
with my employer. All of my performance
evaluations were missing and my
individual development plan. Nothing
about my raise. Or review was in it
either.

The law does not require anything in particular to be in your personnel file. In fact, the law does not require a personnel file at all (though keeping one to document employment and issues is a good idea for the employer). The employer can put whatever it wants into the file, and is not required to put in any specific items or information.
