How can I get my name off of a house deed?

My family and I own a house together. I no long reside at the property and want my name detached due to part of the property being leased. The mortgage is in my brother’s and father’s name, however my sister, mother and I all own equal shares. How can I be detached from the property?

A quitclaim deed will release your entire right, title, and interest in the property to your family members, who are the owners of the property.  
The quitclaim deed is signed in the presence of a notary.  The quitclaim deed becomes effective upon being recorded (filed with the County Recorder's office).
