Your mother can convey the property to you by signing a quitclaim deed.  A quitclaim deed releases your mother's entire right, title, and interest in the property.

The quitclaim deed needs to be signed in the presence of a notary, and then recorded.  Recorded means filing the quitclaim deed with the County Recorder's Office.  The quitclaim deed is effective upon being recorded.

The best way for the transfer of legal title to the home that you have written about held in the name of your mother but you have made all payments on it is for her to sign a quitclaim to it where your name is placed on title and the quitclaim is then recorded with the county recorder's office where the property is situated.
