How does insurance coverage work after a divorce?

My divorce was final 4 months ago and he still has not submitted paperwork to his HR dept at work.I am still under his insurance and have been to the doctor recently for knee surgery. Is this illegal? Will the insurance look at the actual court date when the divorce was final or will they look at the date that his HR submitted the paperwork to announce he is divorced and to take me out of the policy?

The health insurance provider will probably deny the benefit for the surgery so I would brace yourself for what is coming next - the bill.  The date of divorce is the date that they will look at for termination of the policy.  Good luck.
