Do you have a copy of the insurance policy? Do you know what company provided the policy? What you need to do is to contact the insurance company and let them know that your daughter wants to make a claim for insurance benefits. They will likely send you a form to fill out. Plus, you will need to send a copy of her dad's death certificate which you can order from the county.
