How do I answer questions from U.S. consular in regards to K-1 visa petition?

The consular is requesting a notarized statement from me explaining why I used a different last name of my own. I never used it only I think a creditor made a mistake on my credit report when they saw my boyfriend at the time co-sign for me. The consular is also asking why I did not include my alias name, which I am not sure what they are referring to, on the I-129f form. I was not aware I had an alias name so how do I answer that. The consular is also asking I provide proof of singleness. I have already provided a document showing I have never married. What else can I give them?

You need to provide all the information in the form of an affidavit where you state all the facts as you know them to be .  You provide the statement, signed and notarized to the US Consulate in response to their request.  If you need additional assistance with that, you can contact me or an attorney in your area.
