First of all, as a general rule paycheck dedcutions cannot be made from an employee's paycheck without their written consent. This is why you are being asked to sign this paper.
As for whether or not you can be forced to sign it, technically the answer is no. You can refuse. However, your employer could terminate you for doing so. In fact they can fire you for any reason or no reason at all. For the most part, employers can set the terms and conditons of employment as they see fit. That is unless doing so constitutes some form off actionable discrimination or violates company policy, an employment contract, union agreement , etc.
