Typically, you must keep your spouse on your health insurance until the divorce is finalized.  After that, they will be removed from your policy; in fact they'll no longer be eligible for coverage.  If it's a group policy, from your employer, they will be eligible for 18 months of coverage under COBRA, but that will be their own coverage which they will typically have to pay for themselves.

Note: If you gave a separation agreement, see if it addresses the issue of continuing coverage for your spouse.
