Every carrier must provide a complete list of how they cover their prescriptions: as generic, or formulary, or brand-name nonformulary. (Sometimes called Tiers 1,2, and 3 respectively.) Complete open enrollment begins November 15 and ends December 31 of every year. If a company makes a mid-year change that causes a hardship, you are entitled to apply for a hardship enrollment change and move companies.
