When does FMLA officially start?

When does FMLA start? Is it the date the employee notifies the employer (given the proper certification is provided) or is it the date it was certified by the physician. Also, can time be retroed back prior to when we were notified if a doctor certifies the dates?

When did the employee begin taking leave?  Generally, this triggers the act.  Some exceptions apply but your question does not give enough information.
