Is it required that all drivers must have personal injury covered in their auto insurance?

How big is the scope of this requirement, if it is?

Yes, in your state, personal injury protection, or "PIP," insurance is required. You need to purchase a plan as part of your auto insurance which will provide up to $10,000 coverage per person in your car (you and your passengers). This is in addition to the liabiltiy coverage you must also purchase.
