I am so sorry for your loss.  Generally you can file a Probate Petiton listing your wife as "Mary Smith a/k/a (also known as ) Mary Black" so that the court is aware of the other names.  You will likely also need an affidavit from someone who is not likely to inherit from the estate that can give an accounting of her lineage in some way and explaining the marriages.  But I would ask the insurance company what they want from you - in writing - so that you know what will be required.  Good luck.
