If your mother made below a certain amount per year, she may not have needed to file. Yes it will be an issue if she has not filed for 12 years and that will impact family home care applications. You should immediately meet with a CPA (Certified Public Accountant) and see what can be done to negotiate this matter with the IRS.

Just because your mother has not filed income taxes for the past twelve years you will not get in trouble with respect to the application for family home care. However, she should consult with a tax attorney so that she can get her tax filings up to date.
