If I signed an NDA for a company that I no longer work for and I don’t have a copy of it, do they legally have to give me one upon request?

Yes they are required to give you one so you have notice of what you can and cannot disclose. If the company refuses and you wind up disclosing something, you should have this refusal in writing so the fault will be shifted theoretically to the company.  Consider talking to an attorney if this former employer refuses to give you a copy of the NDA.  Of course, you should be concerned with whether or not this was indeed the same version you signed or if this company will attempt to swtich out some documentation.
