What to do if I’m attempting to obtain copies of radiology and surgical notes?

I have reviewed the medical records and the actual operating room suite notes are not apart of the record. Nor are the radiology suite notes. I would like copies of those notes.

If you have filed a lawsuit, you can obtain them using a document production request or written interrogatories (written questions). However, until and unless you actually institute a lawsuit, the health care providers (e.g. the hospital) is  under no obligation to provide you this material.
