My mother has a house in PR and her husband died 12 years ago. His name is still on the title deed.

The best way to get the title isue with resoect to your mother's home resolved is for her to consult with a Wills and trust attorney. If the property was held in joint tenancy, then tranferring title should not be too hard via an affidavit of surviving joint tenant and a certified copy of the death certificate recorded on the property. 0therwise, you might need a probate, trust administration or intestacy proceeding.
