Can I state who will make decisions?

I am estranged from my family and would like to make a directive that if my husband is unable to make decisions for me, I DO NOT want my existing family to have any rights to make decisions. I would like to state another person… as in that person to be considered my only next-of-kin…. Can I do this? and how?

A durable power of attorney for healthcare will allow you to designate the person of your choice to make healthcare decisions for you if you are unable to make those decisions.
At the law library, ask the librarian to help you find a durable power of attorney for healthcare so that you can include the appropriate language when you write yours.
