If 5 days after having the rotor replaced on my car the wheel fell off while going down the road, is the repair shop responsible for damages?

The wheel studs sheared off.

If you can show the repair done by the repair show is faulty and was the direct cause of your wheel falling off, you can hold the repair shop liable for your damages and any repair necessary for your vehicle.
