What do I do with a W-9 form that I received?

What is the W-9 form for? Got one in the mail, and I don't know what it's for. Do I have to send it off?

This form is sent by a company or other employer that you did work for (as an independent contractor, consultant, freelancer, or other self-employed worker). You need to fill out the form by providing your name and social security number (or employer  identification number of your business). Then send it back to the employer. The employer needs it for its tax records.
