I am not admitted in Washington. I would strongly suggest that you have a lawyer in your area look over everything yu intend to file just to be sure.
Here is the Washington statute regarding deianl of paternity:
26.26.355 - Forms for acknowledgment and denial of paternity. (1) To facilitate compliance with RCW 26.26.300 through 26.26.350, the state registrar of vital statistics shall prescribe forms for the acknowledgment and the denial of paternity. The acknowledgment of paternity shall state, in prominent lettering, that signing the acknowledgment of paternity is equivalent to an adjudication of paternity and confers upon the acknowledged father all the rights and duties of a parent, such as the payment of child support, if the acknowledgment is not challenged or rescinded as prescribed under RCW 26.26.310 through 26.26.340. The form shall include copies of the provisions in RCW 26.26.310 through 26.26.340.
(2) A valid acknowledgment or denial of paternity is not affected by a later modification of the prescribed form.
