The Statement of Financial Affairs is part of every bankruptcy petition.  In Question 5 of this ("repossessions, foreclosures, and returns") you will give the particulars about the matter.  If the foreclosure is complete, the lender should be listed with other unsecured creditors in Schedule F.  You will also need to answer the additional business questions (# 19-25) on the Statement of Financial Affairs.
