My Dad is 77 and the only name on Mortgage and title to house. He wants to put my name on the deed/title. How do we accomplish this? Thanks.

He can "quitclaim" his interest in the house to you.  This will transfer title into your name.  You need to fill out a quit claim deed.  Make sure it is a form that is acceptable in your state; you can find them them online.  Also, make sue that you properly fill it out, have it notarized, and recorded in the land records.  You can ask the clerk at the recorder's office for help with this.
