Is my old insurance company liable for treatment for a condition treated during having that insurance?

Question Details: My wife haad back surgery two years ago while we were on Humana Ins. She has been presribed pain medication since then. We now have BCBS since September this year. My wife needs to get off the medication and find a way to control the existing pain.I understand pre-existing conditions and BCBS will not cover the referral from the doc to go to a pain management clinic, but does Humana have some responsibility with this issue? will someone help my wife!

Typically, health insurance policies are only liable for claims/medical treatment during the policy period.

Has BCBS confirmed writing they will not cover the referral?   What does the insurance policy say about this?
