I was on short term disability from work. It was terminated after 5 weeks. I'm appealing the decision.
Question Details: I have a limited time for my appeal. I've been trying for two weeks to get my doctor to send in the paperwork. Even though he charges me for filling out paperwork, they keep telling me they're going to send it in, but i'm still waiting. I haven't received a check for over a month, and my medical insurance was cancelled. Is there anything I can do to force him to submit the paperwork?
