I owe N.Y. state personal back taxes. The NY state tax representative wants my banking info, do I have to disclose that?

Question Details: I am a real estate agent. New York State recently garnished 10% of my wages on a commission check. The NY state tax representative came to job. He wants me to fill out this financial disclosure form. He also asked where I do my banking. I said I would rather not give him that information because I have very little money and he would confiscate my rent money. He said that my not willing to disclose that information hurts my case. If I give them that information, they will clear out my account. What can I do legally to protect my basics needs, like rent, food, and utilities?

I do not know if you quqlify for a chapter 13 bankruptcy.  If you do qualify, the tax debt can be paid through the court over a period of up to 60 months,
