In most states, there isn't any specific form that's needed for an agreement dividing household items and other personal property.  In some cases, it's done with a list of everything, showing who gets what, or two lists, one for what each person is getting.  If there's really no problem at all and some trust left (or the property has already been removed by the people), sometimes the agreement just says that the personal property has been divided up and each person will keep what they have;  sometimes, that language is used with a small number of exceptions, things yet to be turned over.
