If he is not following the medical support order, file a motion to show cause. While there is a court order, your daughter needs to follow it. You can file to modify said court order if you have evidence that it would be in your child's best interests. However if your daughter is suicidal then you need to immediately take care of her needs. The threat to hurt herself or others (the stepmother) is something that should have been dealt with by taking her immediately to the hospital.
