Can Medicaid come back on family members for payemts?

Question Details: My grandma is a full time resident in a nursing home. My grandfather signed their house over to the state of Colorado when applying for medicaid. The equity in the house is running out. Can medicaid come back against family and our houses, business to pay for the overage? I heard that If I sign or pay for anything for my grandparents such as making of a will then I am automatically legally financially responsible. I also heard that since I am family that I am responsible financially if I hold any property.
