Can a company deny insurance for a medical bill that was incurred before you resigned from your job?

I am a teacher. I worked for a charter school. I turned in my resignation 4 months ago on the 5th of the month. I went to the doctor on the 2nd and they denied the insurance payment. They listed my end of employment date as the month prior. Can they legally do that?
No, they cannot simply "back date" your resignation or termination of employment. They could certainly have held your resignation effective as of the moment you handed it in--for example, if you gave 2 weeks notice on the 5th, they could have ignored that notice and held that your job ended on the 5th--but they can't go back and claim your employment ended during periods when you were actually and demonstrably working. If you were working on the 2nd, they would therefore seem to owe you your benefits for that date.
